Hello, Do you have any templates or use cases for a service based company whose only goal is to get an appointment? Is this a lead form that is attached to a Facebook ad? I looked at your site but didn't see anything.
Yes, you can create a survey to collect names, emails, and phone numbers for appointment scheduling. Use the “Name, Email, and Phone Form” template, which you can customize as needed.
You can also include the link to your survey in Facebook Ads.

I understand some new features will be offered at an extra subscription. I would like to ask whether the following features be included in our LTD plan or not. - Quiz feature (adding points for correct answers, etc) - Uploading files from survey - Collecting signatures - Adding images for types of questions
Thanks, and loving the product so far, but I'd hate to get request a refund due to these features not being included in the LTD.
Can't you commit to including any of the above to the LTD? Similar apps on appsumo offer these features at a similar price to MetaSurvey, so as much as we love MetaSurvey, we'll have to request refunds unfortunately.

How can I easily send participants their responses to the survey/assessment when they complete it?
You can use webhooks + automation tool like Zapier, Pabbly, or Make to retrieve respondent emails and survey results, and then send the results to respondents.
